If you are a new patient to our office, the attached file contains our
new patient bundle with forms that will need to be filled out when you
arrive at our office. Downloading and filling them out electronically
and emailing them back to us when you're finished, will allow us to
attend to your dental needs more quickly than
completing them on your arrival. Thank you and please contact our office

We are not a participating dentist in any DMO plans. We will work with
the patients insurance companies to get them back the maximum benefit
that their insurance allows. Patients are responsible for their
payments.
