If you are a new patient to our office, the attached files contain our
new patient bundle with forms that will need to be filled out when you
arrive at our office. Downloading them, printing them filling them out and bringing them back to us when you're finished, will allow us to
attend to your dental needs more quickly than
completing them on your arrival. You can even scan your completed forms and mail it to lekhatulldds@gmail.com. Thank you and please contact our office
